Tissue expansion is a unique procedure used to cover and remove the wide and ugly scars of skin on face, neck and scalp region. Skin expansion by tissue expander is the name given to generation of extra skin by mechanical and biological over stretch. This is accomplished by placing an expandable balloon implant underneath the skin.

Tissue expansion requires minimum two surgeries. In this, a silicon balloon like implant is placed under the normal skin just adjacent to the scarred skin. This implant is expanded at regular intervals until desired area of skin is obtained. In next surgery, the expander is removed followed by removal of scarred region and its replacement by expanded normal skin.

This technique is superior to all other techniques of scar revision because skin from the adjacent area is used to cover the defect therefore there is excellent match of skin color, texture and thickness with minimal risk of rejection and only single suture line along the scar margins. Uses of Tissue expanders

  1. Most commonly used for wide scars over face and neck region.
  2. In treating scars of these scalp or baldness by expanding hair bearing skin.
  3. For correction of various deformities over different regions of body.
  4. In breast reconstruction, expanders are placed underneath the skin of the chest and the native skin is expanded till the definitive implant or flap reconstruction is done to give the desired contour and size.
  5. Can also be used to expand skin on the back or the buttocks, for later transplanting it to other distant body part by microsurgery.

Disadvantages of Tissue Expansion

  1. It is a Two staged procedure, rendering patient to surgery and anesthesia stress twice
  2. In between two stages, patient's expanded body part is disfigured by expansion leading to social discomfort

After the second procedure is done, the results are very satisfying to the patient. Regular follow up is done to assess proper maturation of surgery scar.

Procedure of Tissue Expansion

It is a two staged procedure. The first stage where expander is inserted adjacent to the defect and the second stage where expander is removed and the flap advancement is done.

Patient is discharged usually a day or two after the first surgery of expander placement and then stitches are removed at 12th day approximately. After 2 weeks, expander is begun to inflate. Inflating is done by normal saline fluid at weekly intervals and once the desired expansion is obtained in usually 8-12 weeks, the patient is again admitted for second stage surgery to remove the expander and use the expanded skin the desired way. Post operatively, the patient stays in hospital for 2- 3 days and the course of treatment is same as before except that regular monitoring of expanded skin flap is done.
