How common is cleft hand/foot?

Cleft hand is seen in less than 5 percent of all congenital hand abnormalities. The incidence is roughly 1 in 1,00,000 live births.

The condition can be seen in one hand or both the hands. The child born with cleft hand may also have cleft foot.

What causes Cleft hand/foot?

Cleft hand/foot is seen in babies after birth. The exact cause is not known butis due to the abnormal development of hand or foot during pregnancy.It can be genetic also.

How is cleft hand/foot diagnosed?

The condition is sometimes not detected on a routine prenatal ultrasound. Therefore,deformity of the hand or foot is seenafter the baby is born.

  • Physical examination- center portion of the hand is missing a finger or fingers and there's a V shaped cleft in that area
  • Xray of hand/ foot
  • Other congenital anomalies can be associated with it like cleft lip and palate, deformity in heart or brain.
What are problems children face with cleft hand/foot?

Usually, children with cleft hand are very well adjusted and do not have any functional deformity even when a part of their hand or foot is missing. If the deformity is severe, it will lead to functional limitation forcing the baby to not use the hand or foot well.

How is cleft hand/foot treated?

Parents of children with no functional deformity only want cosmetic correction which is best possible with operative management. Remember occupational therapy is still very important after the surgery. Children with functional deformity will also require surgery.

How is surgery done for club hand/foot at Kayakriti?

Cosmetic surgical correction is usually performed after the age of 1 year.

  • The cleft is closed and functional status of hand is ensured
  • 1st web space (space between thumb and index finger) is adequately created
  • Local flaps are utilized to close the cleft. If required use of regional and distant flaps can be done
  • Stabilization of hand/ foot bones is done using k-wire fixation.

Recovery after surgery for Cleft hand/foot

  • Immobilization with splint for 6 weeks
  • K- wire is removed after 6 weeks
  • Physiotherapy is started with range of motion exercises with night-time splintage for next 3 months.
