Chemical burns are injuries to the skin, eyes, mouth, or internal organs caused due to contact with an irritant chemical. It may be acidic or caustic. Chemical burns can happen at home, public area or at work as result of an accident or an assault or misuse of hair, skin, and nail care products.

Based on the amount of damage to the skin, they could be:

  1. Superficial or first-degree burns
  2. Partial thickness or second-degree burns
  3. Full thickness or third-degree burns

Following common precautions and educating the people can reduce risk of injury.

Household products that can cause chemical burns, including:

  1. Products that contain Ammonia like hair colour
  2. Battery acid
  3. Bleach
  4. Concrete mix
  5. Drain or toilet bowl cleaners
  6. Metal cleaners
  7. Pool chlorinators
  8. Tooth-whitening products

Infants and elderly are most at risk for burns.

All chemical burns are considered medical emergencies and most of these occur on the face, eyes, arms, and legs.

Symptoms of Chemical burn

Patients with chemical burn will experience following problems over the affected area:

  • Redness, irritation, or burning
  • Pain or numbness
  • Formation of blisters
  • Cough or breathlessness

Apart from skin, in few cases, where chemical has penetrated deep- deeper structures can also be damaged. This deeper burn occurrence depends on:

  • Strength of the chemical
  • Contact site(eye, skin, mucous membrane)
  • If chemical is swallowed.
  • Amount of chemical
  • Duration of exposure
  • Chemical nature of the chemical

In serious cases, following may occur:

  • Low blood pressure
  • Weakness, dizziness
  • Breathlessness and severe cough
  • Headache
  • Muscle twitching or seizures
  • Irregular heartbeat and cardiac arrest
  • Rare event of death also

Treatment of Chemical Burn

Assessment of the extent of a chemical burn is done and treatment is begun.

Patient history includes the relevant information regarding the accident scene like

  1. Where did it happen?
  2. How did it happen?
  3. When did it happen?
  4. Name, strength, and quantity of the chemical causing the burn
  5. Even for very small exposure, basic first aid is given. The area involved is inspected. A tetanus vaccination shot is given to all those who are not vaccinated previously or have received vaccination more than 5years ago.
  6. Most people with chemical burns do not need admission and go home and come for follow-up care. In severe cases, however, they may need to be hospitalized.

First aid

  1. Remove the person with the burn from the accident area.
  2. Remove any contaminated clothing.
  3. Wash the area to dilute or remove the substance, using water for at least 20 minutes.Gently remove any solid materials
  4. Especially wash away any chemical in your or the person's eye.

Medical treatment in the hospital

  1. IV fluids and pain killers to treat pain
  2. Antibiotics
  3. Irrigation with water
  4. Antidote is administered if the chemical composition is known.
  5. Regular dressing of the area with creams.
  6. Consultation with other medical specialists.
  7. If there isbreathing problems, a breathing tube is placed in airway.

Surgical Management

  1. Minor chemical burns will heal on its own or with regular dressings
  2. Chemical burns involvingpartial thickness of the skincan also heal with regular dressings. They might require skin grafting at a later stageif the area is covered with a red granulating wound with regular dressings.
  3. For deep full thickness burns of the skin, excision of the burnt area and skin grafting of healthy skin from thighshould be done to replace the damaged skin.

  5. Physical and occupational therapy.
  6. Counselling and support groups can help with emotional trauma from the disfigurement.

Complications of Chemical Burn

Serious chemical burns can cause long-term complications:

  • Pain and scarring.
  • Burns in the eye can lead to blindness.
  • Swallowing can lead to problems in gastrointestinal tract.
  • Loss of fingers or toes.
  • Emotional issues including anxiety, depression, and insomnia.
