Polydactyly is a condition in which a child is born with an extra finger or toe and has more than 5 digits in hand or foot.

This extra digit is usually small and abnormally developed. It is usually of cosmetic concern to the patient or to the parents of the child born with an extra finger or thumb.

What is the incidence of Polydactyly?

Polydactyly is one of the most common hand a nomaly present at birth in babies and the incidence is 1 in 500 to 1000 live births.

What causes Polydactyly?

During the development of the baby inside the mother, baby's hand is in the shape of a paddle which later splits into individual fingers. If the process of separation of the fingers takes more than the required time, then an extra digit is formed.

What are the types of Polydactyly?

A. Polydactyly (extra finger) of handcan be on the

  • Thumb (radial),
  • 2nd, 3rd and 4th finger (central) and
  • Little finger (Ulnar)

Thumb polydactyly is more common in Asians and little finger Polydactyly in Blacks.

Thumb polydactyly is mostly isolated, whereas syndromic association is more common in 5th digit polydactyly.

What are the symptoms of Polydactyly?

This disorder is easily detected due to its cosmetic appearance to parents and the child him self later in life. The child is usually very well adjusted and this extra finger or thumb does not cause any impairment of hand or foot function. However, depending on level of duplication, extra thumb can cause functional problems.

How is Polydactyly diagnosed?

Polydactyly is routinely detected during prenatal ultrasound. Once the baby is born, extra finger is noted on hand or foot and a diagnosis of Polydactyly is made.

Extra finger is usually small and can be composed of-

  • Skin and fat only – mostly seen in extra digiton the little finger
  • Skin, fat and bone
  • Skin, fat, bone and joints also - mostly seen on the extra thumb

In central polydactyly, the second, third and/or fourth digit can be involved in the duplication. Most frequently affected is the ring finger, followed by the long finger and then the index finger.

X Ray is done to see bone and joint of the extra finger and the adjoining normal finger.

On careful observation of the extra digit and its muscle attachments, it is usually noted that-

  • Extensor apparatus is weak,
  • Fingertips can be asymmetric and
  • Nails are smaller on the extra digit
  • First web space is mostly normal

How is Polydactyly treated in children?

The best treatment for Polydactyly is by surgery. The extra digit is removed and the closure is done according to the type and location of the extra digit.

How is Surgery donefor Polydactyly at Kayakriti?

Polydactyly is treated surgically when the child is of minimum 1 year of age.

Thumb polydactyly
  • If it is simple polydactyly (without any bony or tendon attachments), then simple excision of the extra digit is done.
  • If the extra digit and the thumb share common bone, joint or tendons apparatus, then simple excision of extra thumb is not done. One thumb is built or made from both the thumb and the extra digit.
    • Tendons, ligaments are preserved to align and balance the thumb and skin is preserved to augment the newly made thumb.
    • Articular surfaces are aligned as axial, ligaments are reattached, and tendon insertions are balanced.
    • Skin cover is given.
Little finger polydactyly
  • Floating little finger type is excised at its base and the nerve/vessels are buried to prevent painful neuromas in the scar.
  • If a more developed extra little finger is present (share common joint and tendons with the normal finger),then the same principles of reconstruction as in thumb polydactyly are applied.
2nd, 3rd, 4th finger polydactyly
  • Similar approach is used to align the bones and the articular surfaces
  • Balancing ofthe available tendons is done

Recovery after surgery in Polydactyly

  • Hand is Immobilized for 6 weeks
  • Night-time splint for three months
  • Full range of motion with hand exercises
  • In floating type- little finger polydactyly, dressing is removed after 2 days and no immobilization required.
