A surgical procedure which is used to reshape the region of the chin either by augmentation or reduction of the bone is called Chin surgery or genioplasty or Mentoplasty. It is very commonly done in men and women for the under projected and over projected chin and also for the various deformities of the chin like asymmetrical chin, double chin, jawline deformities and as a part of surgical rejuvenation of the aging face. The impact of the chin surgery is tremendous on the facial profile as it enhances the contour and shape of the face.

A youthful face has cheeks which are full and high with well-defined jaw line. This is also called triangle of youth with the base of triangle at the cheeks and the tip of triangle pointed towards the chin. With age, we start to experience facial bone loss and sagging of skin making our face more oblong, cheeks flatten and jowls prominent giving an appearance of inverted triangle or aging triangle. Chin and jawline are the extremely important areas to treat for a youthful appearance. Unfortunately, these are probably the least requested areas because patients are often unaware about the benefits of treating the loss of definition in the jawline and chin area and the amazing, refreshing results they get after that.

In this surgery, surgery is performed on the lower jaw line and chin area to improve the contour of face or proportion of face to improve the confidence and self-esteem of the person. Sometimes chin surgery is an individual procedure and sometimes it is done with other procedures to improve the contour like with Rhinoplasty, face lift, neck lift and many more.

Double chin can also be corrected by advancing the chin bone forward. In people who donot want a surgical procedure on the bone in chin surgery but still want to augment their chin region, it can be done with the help of fat transfer non surgically. Preparations to be done for the surgery

  1. Get the tests doneprescribed by Dr Amit Agarwal.
  2. Monitor your current medications after discussing it with your doctor.
  3. Stop smoking at least two weeks prior to surgery.
  4. Do not take any anti-inflammatory drugs, aspirin or any herbal supplements as they can aggravate bleeding.
  5. Admission will be done on the same day at the centre.
  6. Written consent form will be signed explaining the procedure after the admission.
  7. Meeting with the anaesthetist for Pre-Anaesthetic check-up (PAC) before the surgery if you planning to undergo the procedure under general anaesthesia.
  8. Nil per oral for minimum of 5-6 hours before the planned procedure.
  9. Markings will be done before the surgery.

Chin Surgery procedure includes

Chin surgery can be done by transfer of the fat or by surgery on the bone. For correction of minorflaws of the chin requiring minor chin augmentation, fat transfer can be adequate but if the correction required is more, surgery on the bone should be performed ideally to get better results.

  • Fat transfer to the chin can be done under local anaesthesia. The fat is harvested from other areas of the body with excess fat by liposuction. The fat is then processed, purifiedand injected in the deficient areas of the chinto augment it.
  • Bony chin surgery is done under general anaesthesia routinely. Incision is given inside your mouth on the lower jaw anteriorly to access the bone and a pocket iscreated. The further procedure depends on the choice you've made during discussion with Dr Amit Agarwal.
  • If your chin needs to be augmented, then it can be done by either placing the customised synthetic implant or by advancing the bone forward. Dr Amit Agarwal prefers to do advancement of the bone rather than placing implant on the chin but the final choice will be yours. Customize synthetic implants are available in various sizes and shapeswhich can be inserted in the created pocket and fixed to the chin bone.
  • If your chin needs to be reduced, lengthened or be made symmetrical, then ostetomy or cutting the bone is the only option or choice. For cutting the chin bone, Dr Amit Agarwal makes a horizontal cut along the bone and subsequently moves the chin bone forward, down or backward as per the requirement of the case and fix it with the metal plate and screws. Even asymmetric chin can be corrected by cutting the bone and sliding the chin. Chin can be made longer in short & small chinsby creating a gap between the cut bone and the jaw and fixing it with the plate and screws. The gap usually fills up with the new bone.

The incision is finally closed in two layers with absorbable sutures inside your mouth. These suturesaren't visible to anyone after complete healing.

Recovery after the surgery

After surgery, the treated area is covered with bandage strapped around your chin. This helps in the compression of skin and also reduce swelling giving you desired shape and improved contour. You can be discharged on the same day or next day after surgery. You can apply cold packs to reduce swelling, but it will usually go away on its own within a few days.

  • You will be encouraged to take liquid and soft diet for next few days to avoid discomfort.
  • Antiseptic gargling should be done thrice a day
  • The sutures are inside your cheeks which are self-dissolvable and do not need removal.
  • There can be swelling and bruising of the chin and neck region which usually subsides in 3 to 4 weeks.
  • In the days following surgery, there will be pain which can be subsided by the pain killers given by Dr Amit Agarwal.
  • Soreness might last for few weeks.
  • You will be made to walk few hours post-surgery or the very following day, depending on your will and wish.
  • You can start exercising after two to three weeks post-surgery.
  • You can return to work depending on your will power and the type of work required between 4-6 days.
  • You can return to your normal lifestyle after 2 weeks of surgery.

There will be some specific instructions given to you by Dr Amit Agarwal which includes

  • About the follow up dates.
  • How to take the medications or application of creams. There are not many medications though.

Risks and complications post-surgery

Chin surgery usually gives desired results, but there can be some risks involved which include

  • If fat transfer is done for chin augmentation-swelling, bruising andchances of irregular contour which settles in few weeks with pressure. Absorption of fat can lead to loss of projection also.
  • If bony chin surgery is done –
  • Risks related to general anaesthesia which is very rare.General Anaesthesia is very safe in present times and with best machines and a qualified anaesthetist, chances of encountering any complication is less than 0.1 percent in a healthy individual. But no one can predict who will land up in complication.   • Rare chances of Infection
  • Loose fixation with platecan result in either relapse or asymmetries. Mild facial asymmetry can happen which might require revision surgery.
  • There can be persistent swelling and bruising in the surgical area which reduces with time.
  • If you have poor blood supply, the healing will be poor (seen more in smokers, diabetes, hypothyroidism and in persons with multiple diseases).
  • Sometimes change in skin sensation is noticed.
  • Rare chances of nerve injury and bleeding
  • Injury to the mandibular tooth roots during the osteotomy or screw placement.
