As you grow older, eyes stretch, and the muscles supporting them weaken which may gather fat above and below the eyes which causes droopy upper eyelids and bags under the eyes.

Blepharoplasty, also known as Eyelid Surgery, as the name suggests is used to give better appearance to the eyelids. This also rejuvenates the area surrounding the eyes. This surgery can be performed both on lower and upper eyelids. It repairs the droopy eyelids and also removes excess skin, fat and muscles. Blepharoplasty is not only used to improve the appearance of the eyelids but also relieves the symptoms if you are experiencing some problems with the eyes as well. Preparations to be done for the surgery

The preparations which are to be done for Blepharoplasty or Eyelid surgery include:

  1. Get the tests done which are prescribed by Dr Amit Agarwal.
  2. Monitor your current medication after discussing it with your doctor.
  3. Stop smoking at least two weeks prior to surgery.
  4. Do not take any anti-inflammatory drugs, aspirin or any herbal supplements as they can aggravate bleeding.
  5. Admission will be done on the same day at the centre.
  6. Written consent form will be signed explaining the procedure after the admission.
  7. Meeting with the anaesthetist for Pre Anaesthetic check-up (PAC) before the surgery if you are planning to undergo the procedure under General Anaesthesia.
  8. Nil per oral for minimum of 5-6 hours before the planned procedure.
  9. Markings will be done before the surgery.

Blepharoplasty procedure includes

Blepharoplasty or eyelid surgery procedure includes the following steps:

  • Generally this surgery is performed under Local Anaesthesia but can be opted to be done under General Anaesthesia if you are not comfortable with Local Anaesthesia.
  • An incision is made in the second step of the surgery. Usually these incisions are made with such fineness so that the scars after the surgery can be concealed with the natural crease of the eyelid region. So, for the upper eyelid, incision is made within the natural crease of the eye lid. This incision allows removal of excess skin. Repositioning of the fat deposits and fat excision is only done in very rare cases. In the lower eyelid region, the incision is made just below the lower eye lash. Redraping of fat present below the eye is done over the cheek in such a way that it gives a uniform and levelled appearance. Excess skin is removed through this incision, with the proper positioning or removal of the excess fat.
  • Third step includes the closing of the incision with the help of sutures. These sutures will be removed after a week of the surgery. Usually during the eyelid surgery or blepharoplasty, cold compress or the lubricating ointment is applied. Sometimes it is covered with gauze dressing, after the surgery is finished.
  • Fourth or last step includes observation of the result, which can only be seen clearly after the swelling and bruising subsides.

Blepharoplasty

Precautionary instructions will be given which includes

You can be discharged on the same day or next day after surgery.

  • There can be substantial swelling and bruising of the eyelid region which usually subsides in a week.
  • Sutures are non-absorbable, they would be removed after a week. Dr Amit Agarwal does not prefer to be use absorbable sutures for eyelid surgery as in few cases granuloma might be formed delaying healing process. But on request, absorbable sutures can be placed which does not require to be taken off
  • There should be no vigorous exercises after the surgery until the surgeon recommends. Always keep your head elevated over pillows while lying down on bed or preferably by placing a folded mattress or quilt under the bedding. Ice fomentation can be done.
  • In the days following surgery, there will be pain which can be subsided by the pain killers given by Dr Amit Agarwal.
  • Soreness might last for few weeks.
  • You will be made to walk few hours post-surgery or the very following day, depending on your will and wish.
  • You can start exercising after four to six weeks post-surgery.
  • You can return to work depending on your will power and the type of work required between 2 -3 days.
  • You can return to your normal lifestyle after 2weeks of surgery.

There will be some specific instructions given to you by Dr Amit Agarwal which includes:

  • How to take care of the surgical site.
  • About the follow up dates.
  • How to take the medications or application of creams. There are not many medications though.

There will be some amount of swelling and bruising which will slowly subside. There will be some amount of discoloration, discomfort and dryness which can be controlled with ointments, medications and sometimes cold compress.

The Risks and complications of Blepharoplasty include

There can be some amount of swelling and bruising which usually subsides with time. There can be dryness in the eyes which reduces with drops. There also can be some sensitivity to sun or bright light in the early days after surgery. These are all temporary aftermaths of the surgery which reduces as the time passes by.

  • The surgery is usually performed under local anaesthesia. On request it can be done under General anaesthesia. The Risks related to general anaesthesia are very rare. General Anaesthesia is very safe in present times and with best machines and a qualified anaesthetist, chances of encountering any complication is less than 0.1 percent in a healthy individual.   • Rare chances of Infection which usually subsides with good antibiotics.
  • There can be persistent swelling and bruising, ocular discomfort in the surgical area which reduces with time.
  • If you have a poor blood supply, the healing can be poor giving rise to wound healing problems (seen more in smokers, diabetes, hypothyroidism and in persons with multiple diseases).
  • Rare chances of ectropion of the lower eyelid, asymmetry of the eyes, wound dehiscence, scar abnormalities. Over correction of upper and eyelid laxity can lead to few of the mentioned problems. So do not request your doctor for more than appropriate skin removal during the planning session.
  • Epiphora (excessive tearing) can happen which usually subside in few days.
  • Rare chances of bleeding can occur in persons with undiagnosed bleeding tendencies, on blood thinner medications, herbal supplements for weight loss. It can lead to haematoma formation under the eyelid region. Long standing and unrelieved hematoma can lead to blindness also. So, consult your doctor immediately if it happen sin your case. The doctor will drain the blood out of the area after removing a single suture.
  • Possibility of revision surgery if the results don't meet your desired expectations.

Dark circles can be diminished after the lower eyelid surgery, they surely will reduce but perfect result can only be obtained if a separate procedure is combined with blepharoplasty suggested only to you after your consultation with the doctor.

Will there be any kind of vision loss after the surgery?

If you have an intact vision, the chances of changed vision are negligible. So, you should not worry about it and opt for the surgery./p>

Will I have to be admitted at the center for this surgery?

In normal conditions, we send the patient home on the same day of surgery but if your surgery is a part of other surgeries, this will be best advised by our doctor.

Will there be scars on my eyelids after the surgery?

Usually the scars are hidden under the natural creases of the eyelids, so there won't be a prominent visible scarring.

For how many days will I not be able to open my eyes?

You will open your eyes on the very same day on our doctor's advice.

Will there be long term pain after the surgery?

No, there won't be any long term pain after the surgery. There will be medications given to you by the doctor after the surgery to reduce the on-going pain which will eventually subside once you become stable.
