What are the Preparations to be done for the surgery

Get the tests prescribed by Dr. Amit Agarwal. All pre-surgical workup investigations should be shown to the operating surgeon and the anesthetist, and we will evaluate your fitness for the surgery. You will be given a date for the surgery only after you are declared fit for the procedure

How to get fit a facelift?
  1. Monitor your current medications after discussing them with your doctor.
  2. Stop smoking at least 4 weeks before surgery.
  3. Do not take any anti-inflammatory drugs, aspirin, or any herbal supplements as they can aggravate bleeding.
Other preparations
  1. Admission will be done on the same day at the center.
  2. A written consent form will be signed explaining the procedure after the admission.
  3. Meeting with the anesthetist for Pre Anaesthetic check-up (PAC) before the surgery.
  4. Nil per oral for a minimum of 5-6 hours before the planned procedure.
  5. Markings will be done before the surgery.

MACS Facelift procedure includes

MACS facelift is antigravity vertical lifting of deeper tissues of the face. This surgery is usually done under general anaesthesia to avoiddiscomfort associated with lifting and pulling of the face muscles. The procedure can also be done under local anaesthesia in few selected cases. The best choice of anaesthesia will be recommended by Dr Amit Agarwal to you.

MACS Facelift incisions are then made after infiltration of tumescent solution under the skin flaps to ease dissection and control bleeding. The length of the incision is decided on thetargeted areas of aging and the degree of lifting required.

The incision usually begins in the hairline at the temple region, continues downup to the inner curvature of the front of theear only. In cases with severe sagging, the incision curves around the lobule to be carried behind to the side of the scalp. Dissection is done deeper to the skin above the muscles and fat. Depending on the requirement, sutures are woven into the muscle to restore tightness and lift them in a more vertical direction.

Difference between a Simple MACS and an Extended MACS

Simple MACS Lift is when two sutures are woven to rejuvenate sagging of the neck and lower face and if a third suture is woven to tighten the mid-face and the lower eyelid, it is called extended MACS Lift. Skin is redraped over the uplifted areas and the excess skin is trimmed near the ear and hairline. Skin is closed with sutures and glue over a drain. The incision lines are finely concealed inside the hairline and within the natural contours of the ear. The final scar blends with the hairline rendering it almost unnoticeable.

The visible results of the surgery are seen after the swelling and bruising subsides.

Post Surgery

After the surgery, dressings are applied to the surgical area. You will be discharged the next day of the surgery after the drains are removed followed by the weekly visits or as suggested by Dr. Amit Agarwal. We may send you home with a drain if there is excess fluid in the body. It might be preferable to remove the drain at a follow-up visit. You will be made to walk a few hours post-surgery or the very following day, depending on your will and wish.

How much time does it take to Recover?

There can be substantial swelling and bruising of the face region which usually subsides in a week or two.

Sutures are non-absorbable, they would be removed after a week. Dr. Amit Agarwal does not prefer to use absorbable sutures for closing facelift incisions as in a few cases granuloma might be formed delaying the healing process.

What to do for faster Recovery?
  • There should be no vigorous exercises after the surgery until the surgeon recommends.
  • Always keep your head elevated over pillows while lying down on bed or preferably by placing a folded mattress or quilt under the bedding.
  • Incision and bandage must be kept dry and specific instructions should be followed about bathing and washing.
  • Ice fomentation can be done but not too much as this can compromise the blood flow and result in skin death.
  • In the days following surgery, there will be pain which can be subsided by the pain killers given by Dr Amit Agarwal.
When can you return back to routine activities?
  • Soreness might last for few weeks.
  • You can start exercising after 6 weekspost-surgery.
  • You can return to work depending on your will power and the type of work required after 2-3 weeks.

Risk and complications post-surgery

  • Rare chances of wound dehiscence, scar abnormalities. So do not request your doctor for more than appropriate skin removal during the planning session.
  • Rare chances of bleeding can occur in persons with undiagnosed bleeding tendencies, on blood thinner medications, herbal supplements for weight loss. So, consult your doctor immediately if it happens in your case. The doctor will drain the blood out of the area after removing a single suture.
