Radial longitudinal deficiency is the most common upper limb longitudinal deficiency with an incidence of 1:5000-100000 live births. It ranges from involvement of entire upper limb to involvement of thumb only.
Upper limb evaluation reveals radial deviation at the level of wrist with some degree of thumb hypoplasia.
- Deformity of hand with wrist perpendicular to the forearm
- absent thumb
- Limited elbow movements.
- Entire radius bone could be absent
Surgical Treatment
- All pre operative investigations, Xrays are done
- Other congenital anomalies are diagnosed
- Do ultrasound, ECHO test is done to rule out other abnormalities
Radiographs are taken at age of 1 year
- Patient is assessed for suitability for surgical intervention at the age of 1 year
- The child is assessed for movements at the elbow joint and function of biceps
- If elbow joint is stiff and there is no function of biceps then observation is advised with passive stretching of the radial side muscles
- If elbow function is intact and biceps flexion is present then centralization procedure can be done. Centralization procedure should not be done in older children with good function or in children with elbow extension contractors who rely on radial deviation
- Soft tissue release with bilobed flap procedure
- If needed angular osteotomy of the ulna bone is done
- Wrist brought to neutral position in both the radial/ulnar and extension/flexion axes
- K- wire placed for stabilization with the wrist in maximum correction and do not cross the ulnar physis
- At 4 weeks, k-wire is removed
- Static wrist splint is given with range of motion exercises
Centralization Procedure
- At about 2 years thumb is made with index finger (index pollicization) if thumb hypoplasia is present
- Genetic counselling for parents is very important
- Role of occupational therapist is crucial
