Correction of wrinkles on face - Natural & Youthful Results

What is the natural process of aging?

A natural part of the aging process for all men and women, wrinkles, appear on the face in areas where the skin naturally folds during different facial expressions. They also appear on parts of the body that receive most sun exposure, such as the face and neck, back of the hands, and arms. They develop due to the skin becoming thinner and less elastic over time. Wrinkles are a natural part of growing older, and they affect everyone.

As men and women get older, their skin becomes thinner, drier, less elastic, and less able to protect itself from damage caused due to various environmental factors like sun exposure, smoking, etc. This leads to wrinkles, creases, and lines on the skin. Facial expressions, such as smiling, frowning, or several others lead to the development of fine lines and wrinkles in all individuals. When a person is young, their skin springs back to 'no lines' or 'fine lines'. As age progresses, the interlacing network of elastin and collagen fibers in the skin loosens up, the skin loses its ability to hold moisture and produces less oil and the cells divide less; as a result, the skin gradually stretches and becomes thin. It, therefore, becomes more difficult for the skin to spring back in different facial expressions as we age, resulting in permanent grooves called permanent wrinkles.

What is the difference between Indian skin and skin of western origin?

Wrinkles affect people of different skin tones and quality differently due to structural and functional differences in the skin. The compact dermis is thicker in the skin of Black and Asian people, which likely protects against facial wrinkles. So Indian skin wrinkle less when compared to the skin of people of Western origin.

What are the different types of wrinkles?

Wrinkles can be of types

  • Fine lines
  • Deeper furrows

There are multiple treatment options for reducing or removing wrinkles for both men and women available with Dr. Amit Agarwal at Kayakriti.

  • Medical treatment with Retinoids and antioxidants is the most effective way of fighting signs of aging such as uneven pigmentation and fine wrinkling. They help in reducing fine lines and wrinkles.
  • Chemical peels like glycolic and lactic acid are mild and helpful for fine wrinkles and unevenness. They can cause temporary irritation. The improvement they produce is, however, relatively subtle. Deeper peels- like TCA is not advised by Dr. Amit Agarwal as TCA peel can cause serious side effects like scarring and change in skin color and is quite uncomfortable sometimes.
  • Microdermabrasion removes a layer of skin with a rotating brush and stimulates collagen formation in deeper layers of the skin. It is also useful for fine wrinkles
  • Laser resurfacing stimulates the skin's production of collagen. It is a good treatment but would require multiple sittings and "down time" to heal your skin.
  • Botox Injections. Some injections, including botulinum toxin, relax muscles that produce the "frown lines" on the forehead, and fine lines around the eyes. Improvement lasts several months and must be repeated to sustain improvement.
  • Fillers that are injected into skin folds, especially around the mouth, to improve the appearance of wrinkles. Injections to fill hollows of the cheeks and temples bring about a lifting of sagging skin, especially around the mouth and lateral neck areas. Some fillers stimulate collagen formation, which improves the skin tone and texture.
  • Fat transfer to the face work on the same principle as fillers but work more efficiently as it has a stem cell rejuvenation effect on the skin. Fat can replace the lost volume and at appropriately selected places reduce the appearance of wrinkles also. Micro fat injection can reduce the appearance of wrinkles for – a) nasolabial folds b) marionette grooves c) wrinkles over cheeks, forehead, and upper lip region. Nano fat transfer is also done for improving the quality and texture of the skin and making facial skin glow, shiny and elastic.
  • Surgeries like Facelifts, brow lifts, lip lifts, and blepharoplasty are very useful procedures for men and women with established and deep wrinkles.

What is the procedure for Correction of wrinkles on the face?

The steps of the procedure would depend on the type of wrinkles and the procedure selected for their correction by you and Dr. Amit Agarwal.

Chemical peels with mild peels like glycolic and lactic acid, Microdermabrasion, Laser resurfacing, and Botox injection are done in a few minutes and are OPD procedures. You are made comfortable over a chair, the chosen procedure is done and you are sent back home.

Fat transfer to face

Correction of wrinkles with fat transfer can be done under local anesthesia or general anesthesia depending upon your choice and comfort level. In the first step, liposuction is done to harvest healthy fat from another area of the body (usually the belly, thighs, or buttocks). This fat is then purified, processed, and prepared for grafting. A fine needle and cannula are used to inject the healthy fat into the wrinkles and those areas of the face that have signs of volume depletion due to aging. To account for the body's natural reabsorption of the injected fat, more fat than required is injected. The part is massaged to ensure the fat is evenly distributed, addressing your initial goals for the procedure, and leaving wrinkles less visible and your face looking healthier and more youthful.

Facelift, brow lift, lip lift, blepharoplasty, neck lift

Removes the wrinkling along with sagging of the skin. The details and the complete procedure can be read on their respective pages.

How does recovery after the Chin surgery work?

Recovery after surgery would also depend on the procedure selected for correction of wrinkles.

  • Botox injections generally do not require a recovery period but you need to be careful to avoid exercises for a few days and preferably leave the treated area alone. Avoid touching, rubbing the treated area.
  • Chemical peeling/microdermabrasion/ laser resurfacing- Your skin will be red, tight or swollen. Follow your doctor's directions for sun protection, cleansing, moisturizing and applying protective ointments to soothen your skin. And avoid rubbing or scratching your skin. Treated areas take about one to seven days to heal after treatment. New skin might temporarily be lighter or darker than normal.
  • Fat transfer to face- You can expect little swelling immediately after the procedure, and that swelling can last anywhere between 3 -6 days. You may also experience some discomfort. You will be instructed to maintain head end elevation for 2 to 3 weeks to avoid swelling. You will be made to walk on the same day of the surgery. You will be discharged on the same day or next day of the surgery.
  • To help with healing, take a day or two off from your daily routine depending on your type of work. During that time, make sure to get plenty of rest, maintain head end elevation and avoid straining yourself physically or emotionally. You can use ice fomentation if the swelling is more. At first you may be a little numb but as the swelling subsides, sensation will return.

What are the risks and complications post correction of wrinkles on the face surgery?

Before going for the procedure, you should always understand the risks and potential complications related to surgery.

  • Botox injections - Most side effects are usually temporary and should fade within a few days. There could be pain, swelling, or bruising at the injection site. Few patients might experience headache, fever and chills. Few side effects are limited to areas of injection like droopy eyelids, uneven eyebrows, dry eyes or excessive tearing when given around the eyes. Injections around the mouth may result in a "crooked" smile or drooling. But they all are caused by the unintentional effects of the toxin on muscles surrounding the target areas of the drug. These side effects may take several weeks to improve as the toxin wears off.
  • Chemical peeling/Laser resurfacing can cause Redness, scabbing and swelling. Rarely, a chemical peel can cause scarring — typically on the lower part of the face. Antibiotics and steroid medications can be used to soften the appearance of these scars. A chemical peel can cause treated skin to become darker than normal (hyperpigmentation)- more common after superficial peels or lighter than normal (hypopigmentation)- more common after a deep peel. A chemical peel can rarely lead to a bacterial, fungal or viral infection also, such as a flare-up of the herpes virus — the virus that causes cold sores.
  • Fat transfer to face for correction of wrinkles usually gives desired results but there can be some risks involved which include
    • Risks related to anaesthesia.
    • There can be persistent swelling and bruising in the surgical area which reduces with time.
    • Rare chances of infection, delayed wound healing (seen in smokers, diabetics, hypothyroidism, very high BMI, or people with multiple comorbidities)
    • Redness and tenderness of skin which abates with time
    • Irregular contour in form of lumps and bumps.
    • Asymmetry due to unpredictable absorption of fat
    • Numbness or altered sensation which improves with time.
  • Facelift, brow lift, lip lift, blepharoplasty, neck lift - the details of the risks and associated complications can be read on their respective pages.

These are the risks and possible complications which should be discussed by you with Dr Amit Agarwal before the procedure.
