What is a gummy smile?

A gummy smile is characterized by an excessive display of gums on smiling. It is a personal aesthetic condition which rests in the eyes of the beholder. It's a condition where more of a person's gums are revealed when he/she smiles, which in technical language is called an excessive gingival display. Though it is a fairly common condition, it is a major hurdle in the overall personality of an individual and is often a cause of low self-esteem and confidence in men and women.

What are the points to be considered in a Gummy Smile?
  • Height, shape & colour of teeth.
  • Movement of lips when one smiles.
  • The angle of the jaw compared to the rest of the face.
  • *3 to 4 mm of the exposed gum line is considered to be disproportionate.
What are the causes of Gummy Smile?
  • Bad Bite - When the upper and lower jaw is not properly aligned with each other.
  • Hyperactive Upper Lip - When muscles of the upper lip and under the nose are repeatedly overused it can cause the top lip to get raised exposing the gum tissues.
  • Excessive Gum Tissue - The most common reason for a gummy smile is the overgrowth of gum tissue during the eruption of permanent teeth.
  • Vertical Maxillary Excess - Bones on the upper jaw grow longer than their typical length.
  • Lip Size - When the upper lip is a shorter size.
  • Medications - Conditions like Gingival Hyperplasia can be caused by drugs preventing seizures, immunosuppressive drugs & cardiovascular medications etc.

Depending on the cause as mentioned above, the treatment procedure is planned. It could be

  • Botox injection into muscle
  • Gum surgery
  • Lip repositioning surgery
  • Orthognathic Lefort 1 surgical osteotomy and setback

What are the procedures for correction of a gummy smile?

Botox

Botulinum Toxin, also called Botox, injections used to paralyze the elevator muscles in the lips. This will help in covering the teeth with the lips and reducing the gummy smile

Oral Surgery

If too much of your gums cover the surface of your teeth, we will recommend you a dental consultation with Dr Shweta Jain. She will do a procedure known as a gingivectomy. This is also known as gum contouring and involves the removal of extra gum tissue.

  • In the first step, you will be given anaesthesia to keep you from feeling any pain during the surgery
  • In the next step, with the use of a scalpel or laser, your gum will be reshaped to reveal more of the teeth' surface
  • After surgery, mild bleeding of the gums and soreness may be present for about a week.
Lip Repositioning Surgery

If your lips are the cause of your gummy smile, Dr Amit Agarwal will suggest lip repositioning surgery. The procedure changes the position of your lips relative to your teeth. Sectional removal of underlying connective tissue of the upper lip prevents the elevator muscles from lifting the upper lips too high above the teeth. This procedure also lengthens your upper lip.

  • The surgery is performed under local anaesthesia
  • Once your mouth is numb, two incisions will be made on the underside of your upper lip and a section of connective tissue will be removed from the area.
  • The incisions will be stitched with sutures
  • The procedure takes about 45 minutes & antibiotics & painkillers are prescribed post-operatively.
  • It takes about a week to recover from the surgery.
Orthognathic Surgery

If your upper jaw is part of the reason for your gummy smile and you have an excessive gingival display, Dr Amit Agarwal will recommend orthognathic surgery for its correction. LeFort 1 osteotomy and setback will balance the length of your upper and lower jaws. Proper planning through a CT scan is required to perform this surgery.

  • The patient will be given General anaesthesia
  • The surgeon will remove a section of bone from your upper jaw to balance the length of your upper and lower jaws.
  • The upper jawbone will be reattached with tiny plates and screws. If your lower jaw sits too far back, it may have to be adjusted too.
  • You may have to wear elastics to hold your jaw in position while it heals for a few weeks.
  • The procedure takes about 2-3 hours & You will be kept in the hospital/centre for 2 days on iv antibiotics & painkillers
  • It takes about a week or two to recover from this procedure.

Recovery after Gummy Smile Correction surgery

  • Recovery time following a gummy smile surgery is a few weeks. Limitations on diet persist for two weeks as does restriction on regular oral care.
  • Most patients return to work a week or two after the procedure. Complete healing of the gums can take up to three months.
  • You will be able to see the results instantly, however, precautions need to be taken not to stretch the muscles too much which may cause the stitches to loosen or break.
  • A soft and bland diet would be preferred for 7 days after the procedure.

What are the Risks and Complications post-surgery?

  • The lips are among the most sensitive parts of your body, so it's important to work with an experienced surgeon to minimize your risk of side effects. However, there are possibilities for adverse side effects including-
  • Rare chances of infection and bleeding and poor wound healing (seen more in smokers, diabetes, hypothyroidism and persons with multiple diseases).
  • Numbness or altered skin sensation which improves with time
  • There can be pain which subsides with time and slight medication.
  • Risk of teeth loosening and loss over time
  • Prolonged swelling and skin discolouration.
  • Possibility of revision surgery.

These are the risks and possible complications which should be discussed by you with Dr Amit Agarwal before the procedure.

What are the Do's & Don'ts post Gummy Smile Correction?

  1. You should always discuss any previous surgery or medical conditions without hesitation.
  2. You should immediately consult Dr Amit Agarwal if you have any complication.
  3. You should always maintain a healthy lifestyle for best and prolonged results.
