Autologous Breast Augmentation with Fat Grafting

Fat grafting to the breast is kind of an alternative to artificial breast implants. Fat does not have its own blood supply to keep it alive, it relies on the blood supply already in the breast to survive. If you have inhibition of implants being used in your body, fat grafting can be done for you but it can be done in only few selected cases. If a lady has grade 1 ptosis (sagging breasts), fat grafting to the breast is ideal, because a looser pocket allows for a greater percentage of fat surviving, compared to a tight breast or ptosis stage 2 or higher. Since fat does not have its own blood supply to keep it alive as discussed above, so a limited amount of fat can be injected. At most, an A-cup breast will increase to a B-cup. With breast implants, there is no such limitation.

Fat grafting provide a totally natural feel as the breast are enhanced using patient's own fat. When used to augment healthy breasts, the procedure has a good track record, with few complications or need for re-operation as compared to implant-based augmentation.

Dr Amit Agarwal harvestsfat by liposuction from the patient's body and it can only be done from the regions higher in fat deposits like abdomen or lateral thigh region. The fat is then processed and transferred to the different quadrants of the breast. Additionally, the only incisions are the 4-millimeter incisions used to remove (liposuction) and re-inject the fat. The limitations of fat grafting to the breast are-

Gynaecomastia

  1. The amount offat takeupin the breast is quite variable in every patient. The fat transferred to the breast gets reabsorbed by the body upto 30 to 40 to percent. That is why, we at Kayakriti center always overfill the breast. You will feel the desired result achieved immediately after surgery but sometimes with time 20 to 30 percent of projection is lost due to reabsorption of fat. So,sometimes this procedure often requires stages, so it takes minimum of 2-3 sittings to achieve the desired result.
  2. If the patient is skinny, the procedure becomes nearly impossible as there won't be any extra fat which can be transferred.
  3. This procedure does not yield good result in patients with tight skin envelope and with moderate to high grade ptosis.
  4. Fat grafting may interfere with breast-screening examinations. The volume of fat required to enlarge the breast makes the technique prone to calcification which will be visible on a mammogram. These changes will be present over a woman's lifetime.

Breast Augmentation with Implants

A breast implant is a medical prosthesis that is placed inside the breast to augment, reconstruct, or create the physical form of the breast.

There are mainly two types of breast implants available which can be used accordingly.

Brow Forehead Lift

Silicone Breast Implants - They are the most commonly used implants all over the world. These implants are filled with silicone gel. The gel present inside these implants feels like the natural breast tissue so are more natural. They are less likely to wrinkle or rupture. If the implant ruptures, the gel may remain inside the implant shell or may escape in the breast implant pocket butthe silicone filler do not migrate to the other areas of the body eliminating the occurrence of filler leakage "silicone gel bleed". The pro of using these fifth generation silicone implants over its earlier versions is that they generally do not collapse even after leaking because of being filled with semisolid silicone gel. These silicone implants are FDA approved for augmentation in women of age 22 or more. These new generation of silicone implants reported relatively lower rates of capsular contracture and of device-shell rupture, and relatively greater rates of "medical safety" and "technical efficacy" than those of early-generation breast implants.

Saline Breast Implants - These implants are filled with sterile salt watercovered with a silicone shell. These implants provide a uniform shape, feel and are firm. FDA-approved saline implants for augmentation in women age 18 or older.It requires less-invasive surgical technique, by inserting an empty, rolled-up breast implant through a smaller surgical incision and then fill it with saline solution through a one way valve. The surgical incision scars would be smaller and shorter than the surgical scars typical of the pre-filled, silicone-gel implant surgical technique. Saline implants are more likely to wrinkle, fold, rupture or decrease in size over time. The implant is also sometimes noticeable to the eye and touch seen more in women with less breast tissue. In women with good breast tissuesaline breast implants can give an aesthetic result much like that produced by silicone breast implants-an appearance of proportionate breast size, smooth contour, and realistic consistency.

Polyurethane Implants - are silicone gel implant with a coating of polyurethane in the shell of the implant. This coating reduces the degree of capsular contracture by causing an inflammatory reaction which impedes the formation of a capsule of fibrous collagen tissue around the coated device. But still the medical use of polyurethane-coated breast implants is not approved in USA due to the potential health risk posed by 2,4-toluenediamine (TDA), a carcinogenic by-product of the chemical breakdown of the polyurethane foam coating of the breast implant. Though the chances are very rare, polyurethane-coated breast implants remain in plastic surgery practice in Europe and in South America mostly. Dr Amit Agarwal does not use polyurethane implants.

How to choose a correct size of implant

It is very important to choose the correct size implant for breast augmentation. A number of factors determine the best implant size for you can be your desired size, basic characteristics of your breast, chest wall and tissues.

The basic measurement unit for a breast implant is Cubic Centimetre which can be seen on any Implant packaging. They come in a wide range of sizes such, from 80CC and can range up to 800CC.There are different tests that be performed for choosing the correct implant for your augmentation. There are two tests that can be done at home as well for checking the size of the implant.

  1. 1. RICE TEST: This test is generally the most popular test for checking the implant size. This can be done at home. The method to perform the test is with
    • uncooked rice or instant mashed potatoes
    • nylon stocking or plastic bag
    • a sports bra (preferrable).

If you are using a nylon stocking, cut it around 12 inches length excluding the foot part and tie a knot in the endof the leg part. Pour uncooked rice measuring the amount you prefer in the stocking. You can measure the amount with the food scale or a measuring cup. If you are using a food scale, all you need to know is that 1 ounce of rice is equivalent to 30 cc/ gram. For example, 5 ounces of rice is roughly the same size and weight as a 150 cc implant.If you're using a measuring cup (which you will need if using instant mashed potatoes), use the following as a guideline:

    • 1/8 cup rice = 30 cc/gm
    • 1/4 cup rice = 59 cc/gm
    • 1/3 cup rice = 78 cc/gm
    • 1/2 cup rice = 118 cc/gm
    • 2/3 cup rice = 156 cc/gm
    • 3/4 cup rice = 177 cc/gm
    • 1 cup rice = 236 cc/gm

Fill them and stop when you feel that it's the right implant size for you. Make 2 cups to fit in each bra cup. Keep them in your bra and try it with every dress, t shirt and other outfit to check the right size fit for you. If you feel it's not the right size, measure it again from the beginning and then try it again.

  1. 2. WATER TEST: Water test is almost same as rice test but instead of nylon stocking and rice you use water in a plastic back and try it in the same way as the Rice Test.

These tests can give you a good idea of how you would look after the augmentation procedure and what's best is, you can perform them yourself. The other procedure for size test is your sizing session with Dr Amit Agarwal in which you can use real implants during the consultation with your surgeon.

How to choose a correct shape, texture and profile of implant

Surface of implant- Textured OR smooth surface implant?

Dr. Amit Agarwal mostly offers smooth walled implants to his patients. The smooth implants have the advantage of minimal wrinkling. Smooth breast implants are even and soft, enabling them to move freely within the breast pocket. This creates a more natural movement. However, they do have less ability to adhere to the tissues in the chest for proper placement. Textured implants have a rough surface, which enables them to securely attach to surrounding tissue. This ensures they're less likely to move around. Although this may decrease the risk of a tight scar capsule, textured breast implants can still wrinkle. In 2011, the FDA had identified a possible association between textured implants and the development of anaplastic large-cell lymphoma in 0.28percent of the implanted women.

Shape of implant- Round OR tear drop shaped implant?

Gynaecomastia

Dr Amit Agarwal mostly offers round implants to his patients but choice of the shape of implant depends on the desired overall breast shape by the patient. The most popular implant shape is the standard round shape. Round breast implants give the appearance of more fullness at the top of the breast. If you want to correct sagging, and want a fuller look, round implants are for you. Round implants will cost less than teardrop shaped implants. Round implants come with both smooth and textured surfaces. Teardrop shape implants have a more natural look, bottom is fuller than the top of the breasts. As the name suggests, this gives the breasts a more tear-dropped shape. They are growing in popularity but have limited use. Unlike the round implant, the tear drop implant cannot be rotated or moved because it would create an unusual appearance. These implants also require longer incisions and come only with textured surfaces to prevent rotation. They may be ideal for women who want to enhance the lower portion of the breast or women needing breast reconstruction.

Projection of implant- High, medium OR low profile Implant?

American Society of Plastic Surgeons defines a breast implant's profile as "how much an implant projects forward from the chest wall when standing. Implants of the same size (cc volume) that have different widths will produce different levels of projection. Implants with a smaller base width provide greater projection as compared to implants with a wider base width."The implant profilebascially depends on the look you're going for and on the width of the chest

High - High profile implants are the most full and round,and gives the upper portion of the breast extra volume, similar to the look of a push-up bra, making them appear less natural on some patients. This type is ideal for patients with a narrow chest wall. They may separate breasts further and leave a smaller amount of cleavage

Moderate - If you're looking for a profile between low and high, you'll want moderate. It looks the most natural and is great for women with smaller or narrower chests. A medium profile implant tends to have less fullness at the top, but more towards the bottom. The medium profile may allow for more cleavage, but will generally have a more subtle overall appearance

Low - Ideal for women with wider chests, they are relatively flat in appearance, with minimal projection.

Gynaecomastia

Preparations to be done for Breast Augmentation

  1. You should be physically healthy.
  2. You should stop smoking if you are a smoker before at least two weeks prior to the surgery.
  3. Get the tests done prescribed by Dr Amit Agarwal.
  4. Avoid taking anti-inflammatory drugs and herbal supplements as they aggravate bleeding.
  5. Admission will be done on the same day at the center.
  6. Written consent form will be signed explaining the procedure after the admission.
  7. Compression garment measurement should be given for post-surgical use
  8. Meeting with the anaesthetist for Pre-Anaesthesia Check-up (PAC) before the surgery. All pre surgical work up investigations should be shown to the operating surgeon and the anaesthetist, and we will evaluate your fitness for the surgery. You will be given date for the surgery only after you are declared fit for the procedure.
  9. Nil per oral for minimum of 5-6 hours before the planned procedure.

Breast implant is a surgical procedure so you should always have an attendant who can accompany you after the surgery for at least 24 hours and drive or take you home after the procedure.

Breast Augmentation Surgical Procedure

A Breast Augmentation surgical procedure includes following steps:

  • First step is to put you to sleep with general anaesthesia by our very competent intensivist and anaesthetist.
  • In the second step, an incision is made in the unnoticeable areas of your breast region depending on your incision options given by Dr Amit Agarwal. The choice of incision can be-along the inframammary fold under the breast or in the armpit. The choice may also vary with the type of implant or the desired degree of enlargement.
  • Gynaecomastia
  • A pocket is created at the desiredlevel of implant insertion-
    • under the pectoral chest muscle
    • Or directly behind the breast tissue, over the pectoral muscle
    • Or under the muscle fascia below the breast tissue
  • After the pocket is formed,implants are inserted in one these pockets chosen by the Dr Amit Agarwal and you during consultation.
  • After the implants are properly placed in these pockets with full sterile technique, the incisions are closed with layered sutures and skin adhesives

Recovery after the surgery

After the surgery, dressings are applied to the surgical area, and you are made to wear a compression garment to reduce swelling and to support your Breasts. In the recovery period of Breast Augmentation. You will be under closed monitoring for 4-5 hours and will be discharged once you are stable to go home which is usually within 24 hours.

  1. The sutures will be mostly within the breast crease or in the armpit which will be dissolvable and do not require to be taken off.
  2. Mild Pain for around a weekfor which pain killers will be given by Dr Amit Agarwal.
  3. Soreness and mild swelling might last for a week.
  4. You will be made to walk 6 hours post-surgery or the very following day, depending on your will and wish.
  5. You can start exercising after three weeks post-surgery.
  6. Pressure garment is a must wear for the next 3 months without gap or fail for almost 24 hours a day.
  7. You can return to work depending on your will power and the type of work required between 3-6days.
  8. You can return to your normal lifestyle after a week of surgery.

  • Rare Risks related to general anaesthesia-General Anaesthesia is very safe in present times and with best machines and a qualified anaesthetist, chances of seeing any complication is less than 0.1 percent in a healthy individual.   • Implant leakage or rupture - It is seen more with saline implants than silicone implants.If a saline implant leaks, the solutionwill be absorbed and will be expelled by the body naturally. If a silicone-filled implant leaks, the gel will either stay in the shell or escape into the breast implant pocket. A leaking silicone-filled implant may or may not collapse.
  • Risk of developing cancer/lymphoma after Breast Augmentation - The Food and Drug Administration (FDA) in USA has identified a possible association between breast implants and the development of anaplastic large cell lymphoma (ALCL), an uncommon cancer of the immune system. The condition is now known as breast implant-associated anaplastic large cell lymphoma (BIA-ALCL). The FDA believes that women with silicone breast implants that have textured surfaces or polyurethane outer shell have a very low but approximately 0.28 percent risk of developing BIA-ALCL. However, that doesn't mean that these implants cause BIA-ALCL. Further research is needed to understand the relationship between the condition and breast implants. ALCL can develop in any part of the body, most commonly the lymph nodes and skin. Research suggests that BIA-ALCL is usually found next to the breast implant within the surrounding scar tissue, not the breast itself. Treatment involves surgical removal of the implants and the cancer. Sometimes chemotherapy and radiation therapy are needed. When caught early, BIA-ALCL is usually curable.Based on the currently available data, the type of implant fill (silicone or saline) does not appear to be a risk factor for BIA-ALCL, but this has not been evaluated in a large, well-designed, epidemiologic study.
