Burn scar healed by secondary intension sometimes leaves a white patch usually in the center of the scar known as post burn vitiligo. This occurs due to inability of formation of melanocytes, which are skin color pigment producing cellsin the scarred tissue.
This considered as a social stigma bigger than the burn scar itself. Treatment options is mostly surgical.
Medical
Patient usually consult a dermatologist for conservative treatment by medicines and topical creams but there is no treatment for post burn vitiligo by medication or creams.
Surgical
Vitiligo patch is removed superficially by rubbing a dermabrasion burr by method known as dermabrasion. This leaves a raw area which is later covered by two methods. One method is placing an ultrathin skin graft which gives an almost same skin color after healing and advantage is that the patch is treated completely in a single stage. Some patient face a few side effects like itching, small cysts over the grafted area but these are temporary and can be treated conservatively later on. It is considered standard method of surgical treatment for vitiligo.
Second method involves dermabrasion of the vitiligo patch but instead of an ultra thin skin graft, we spread a suspension of skin color pigment producing cells called melanocytes over the created raw area and dressing is done. First dressing is opened on 7th day, at this time, no change in the color is seen. Cultured melanocyte suspension takes 5 to 6 weeks to grow at the donor site and start to give skin color. This technique has advantage of relatively lesser donor site area is required and inclusion cysts do not occur in this method.
Post burn hypertrophic scar/keloid
Keloids are also raised firm nodules forms in the scar but they are more aggressive in the course. They increase in size and cross the boundaries of the original scar. They cause mild symptoms like itching, redness and cosmetic distortion of the involved body part but there is no serious complication.
These can be prevented by pressure garments, massage therapy and silicone gel application. This is also the first line of treatment for hypertrophic scars.
If not relievedby this, intralesional triamcinolone injection is given on the hypertrophic scar or keloid at monthly intervals. This is a highly successful treatment and reduces the size of hypertrophic scar and keloid significantly after few doses. It also relieves itching and redness.
Low dose radiotherapy is also an option for keloids treatment if it is refractory to treatment with injections.
Last resort is surgical excision followed by both low dose radiotherapy and intralesional injections of triamcinolone. Surgical treatment is done in cases which are highly refractory to conservative treatment and is causing symptoms and disfigurement that cause discomfort to patient.
