Temporary Thigh Pouches for Testicular Positioning
Although thigh pouches may offer a short-term solution for testicular positioning, long-term use can lead to pain, psychological issues, and significant abnormalities in sperm count after two years. Therefore, thigh pouches are not recommended as a permanent solution for patients wishing to preserve fertility.
Penile Skin
For up to 40-50% loss of penile skin, primary reconstruction can be achieved using local tissue arrangement and techniques.
Local flaps can be taken from the groin region, pubic area, or prepuce.
Split-thickness skin grafts from the thigh may be used in cases where no other options are available, but it is not the preferred technique.
Scrotum
The laxity of scrotal skin allows for primary closure even in cases of up to 60% skin loss.
When primary closure is not feasible, scrotal and penile skin reconstruction can be accomplished using skin grafts.
Suturing the spermatic cord and testicles together along the midline helps minimize the grafting area and prevents the formation of a double scrotum.
Meshed grafts are suitable for scrotal repair, as they allow exudate drainage and mimic the rugae normally found in scrotal skin when healed.
Local flaps from the thigh or perineum can be used for scrotal and penile skin reconstruction.

Skin grafts:
Lack sensation.
Require an extended and time-consuming wound care period.
Do not contribute to the thermoregulation of the testes.

These advantages include a reduced risk of complications such as skin maceration, poor graft take, and graft breakdown due to fecal and urinary contamination. Additionally, local flaps require less intensive postoperative care.

Various muscle skin and skin flaps derived from the perineum, groin, pubic region, and thigh tissues can be employed for scrotal and penile skin reconstruction. Some commonly used flaps include the medial circumflex femoral artery perforator flap, the gracilis fasciocutaneous flap, the neurovascular pedicle pudendal thigh flap, and the Singapore flap.
