Tissue Expanders for Burn Scar Hair Loss

Tissue expansion is a proven method for addressing post-burn scalp baldness, though it typically involves a two-stage surgical process. Here’s how it works:

• In the first stage, a silicone balloon-like implant is strategically placed beneath the healthy skin adjacent to the bald area on the scalp.
• Over time, the implant is gradually expanded at regular intervals until it provides enough skin coverage for the bald patch.
• The second stage of surgery involves the removal of the expander, followed by the excision of the bald patch. The area is then replaced with the expanded, normal hair-bearing skin.

This approach offers several advantages over traditional hair transplant techniques. By using adjacent skin for reconstruction, we achieve an excellent match in terms of skin color, texture, and thickness. This minimizes the risk of rejection and leaves only a single suture line along the scar margins.

The procedure is conducted in two stages, with the first involving expander placement and the second focusing on expander removal and flap advancement. Patients are typically discharged within a day or two after the first surgery, and stitches are removed around the 12th day. Approximately two weeks after the surgery, the expander is gradually inflated with normal saline fluid at weekly intervals. Once the desired expansion is achieved, usually within 8-12 weeks, the patient returns for the second stage of surgery to finalize the reconstruction.

Hair Transplantation at Kayakriti

Hair transplantation is another viable option for addressing burn scars in hair-bearing areas of the scalp and eyebrows. However, it’s important to note that this procedure presents unique challenges for the surgeon due to the distinctive nature of scar tissue.

• Hair transplantation involves surgically removing hair follicles from a ‘donor site’ and transplanting them to the bald or thinning areas, known as the ‘recipient site.’
• This approach can effectively cover bald patches, provided there is sufficient undamaged donor hair available elsewhere on the scalp for transplantation, without significantly reducing hair density in the donor area.
• Hair grafts can be harvested from both the scalp and the beard.
• In preparation for the surgery, patients are advised to refrain from using medications that might promote intraoperative bleeding, which can affect graft survival.
• Additionally, avoiding alcohol and smoking is essential to ensure the success of the grafts.

Challenges of Hair Transplantation in Burn Cases

In cases of burn injuries, hair transplantation poses specific challenges for the surgeon due to factors like reduced blood supply and altered skin elasticity in the affected areas. The underlying tissues around the hair may be scarred and replaced by fibrous tissue, complicating the procedure. Nevertheless, hair transplantation remains the preferred choice for restoring hair over the eyebrow region in burn cases.

Recovery After Hair Transplantation

Following a hair transplant procedure, postoperative antibiotics are typically prescribed to prevent wound or graft infections.

• Patients are advised to shield the vulnerable recipient area from direct sunlight, and shampooing is usually initiated 5 days after the surgery. This is crucial to prevent scab formation around the transplanted hair shafts.
• During the first ten days after the procedure, it’s common for some of the transplanted hair to fall out due to a phenomenon known as “shock loss.” This is a normal part of the process and should not cause undue concern.
• Over the course of two to three months following the surgery, new hair growth will begin as the transplanted follicles take root.
• Patients can expect their hair to grow naturally, and over the next six to nine months, the transplanted hair will continue to thicken and improve in density.
